GuideStar USA, BoardSource, The Rock Center for Corporate Governance at Stanford University, and the Center for Social Innovation at the Stanford Graduate School of Business are presently conducting a survey on the Board of Directors of Nonprofit Organizations. The goal of this study is to provide insights into the structure and workings of boards of directors of nonprofit organizations. They will provide participants with a summary of our research findings at the conclusion of the study. As a participant in this research, you should read and understand the following:

• Completion of the study is estimated to take approximately 10-15 minutes.
• All participant responses will be kept in strict confidence. No individual company will be disclosed, and your individual privacy will be maintained in all published and written data resulting from the study.
